If you are a new patient, select the appropriate form below. Provide the form to your family doctor or any walk-in clinic to send us a referral. Please have your doctor fax the referral to our clinic via Fax: (289) 891-9591. Once received, we will call you to schedule an appointment at the earliest available date.
